Arapahoe House is currently seeking a Case Manager to join our StepWise team.
The Case Manager will work mostly days, Monday through Friday but hours and days may vary based on client needs. The Case Manager will provide case management services to adolescent clients and maintain contact with parents, case workers, educational institutions, insurance providers, and probation officers by performing the following duties.
Essential Duties and Responsibilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Other duties may be assigned.
- Assists the Primary Therapists in providing assessment, client orientation, safety monitoring, crisis intervention, linkage and service coordination and advocacy for adolescent clients and attend significant meetings as assigned (IE. client staffing meetings, team decision making meetings (TDM), school meetings, county meetings related to client review, etc.)
- Assists in all milieu activities as needed/requested, in accordance with existing staffing patterns, planned special activities and emergencies.
- Maintains accurate records and documentation on clients receiving services.
- Assists in planning, directing and attending outside activities relating to the program.
- Assists the Primary Therapists in conducting clinical updates on all clients.
- Attends and actively participates in treatment review updates on all clients.
- Assists with client intakes, discharges and closing of client records including person and property searches, UA’s, room assignments, etc.
- Provides or assists with transportation of clients to outside agencies and activities including court, social service, school, vocational meetings and other appointments.
- Assists in assuring that required appointments for youth occur within the required timeframes.
- Assists in the aftercare planning for all clients by making referrals/transfers to other programs.
- Participates in and promotes a positive, supportive, cooperative team environment.
- Attends and participates in staff meetings, training classes and supervision.
- Adheres to Policies and Procedures.
- Adheres to Arapahoe House Code of Ethics as well as ethical standards of the field.
- Must be sensitive to the diverse cultural and situational needs of the clients in the program.
Qualifications: The requirements listed below are representative of the knowledge, skill, and/or ability required to successfully perform this job.
Education/Experience:
- Must be at least 21 years of age (TRCCF requirements).
- Bachelor’s degree in a related field (psychology, counseling, human services or social work)
- Minimum of three years related experience or equivalent combination of education and experience.
- Minimum of six months case management and counseling experience
- Minimum of one year experience working with high-risk youth.
Certificates and Licenses:
- Registration in the State of Colorado’s DORA database within 90 days of hire as one of the following approved Addiction Counselor designations: Registered Psychotherapist, Certified Addiction Counselor, or mental health licensure as a Licensed Professional Counselor (LPC), Licensed Marriage & Family Therapist (LMFT).
- Certified Addictions Counselor II (CAC II) is preferred.
- Valid Colorado Driver’s License with a driving record that meets agency requirements.
- This position requires traveling between metro locations.
- Reliable transportation is required.
- Current CPR/First Aid Certification or ability to obtain within six months of hire is required.
- Qualified Medication Administration Person (QMAP) or ability to obtain within six months of hire is required.
Computer Skills:
- Basic computer skills, including ability to use Microsoft Windows, Microsoft Outlook and Microsoft Word and to operate a copier, scanner and signature pad. Proficiency working within specialized software utilized in program.
Language Ability:
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
- Ability to write routine reports and correspondence.
- Ability to speak effectively before groups of clients, community members or employees of organization.
Math Ability:
- Basic mathematical skills – Ability to perform basic math skills, use decimals to compute ratios and percents, and to use and interpret units of measure in regard to medication administration. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Reasoning Ability:
- Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
- Ability to interpret a variety of instructions furnished in written, oral form.
- Ability to respond to and react appropriately in crisis situations.
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
