Founded in 1955, ADMHN provides professional, culturally inclusive mental health and substance abuse services, including outpatient services such as individual, family, marital and group counseling and psychiatry– plus a full range of specialized programs including an acute treatment unit, program for those with severe and persistent mental illness, and more.
The Care Manager 1 works with a client group of older adult population, providing case management and psychosocial interventions. Role is to support quality of life, provide opportunities for therapeutic recreation, support emotional wellness and overall care needs. The Care Manager 1 is responsible for coordinating care with collaterals and family members and other relevant partners as related to older adult needs. This position is primarily community based and requires a high level of autonomy and responsibility.
A. Education: Bachelor degree in mental health or related field.
B. Experience: Minimum one year of experience with adults within a human services setting.
C. Skills: Ability to establish trust and a rapport with older adults and their support systems including but not limited to families, significant others and care takers. Ability to assess crisis situations
D. Other: Ability to lift equipment and supplies up to 15 lbs. Must possess own transportation, a valid driver’s license, maintain a clean driving record (must not have had one or more Type A violations or three or more Type B violations within the past 3 years), and proof of vehicle insurance. Must be able to pass Master Drive driving course as a condition of employment.
A. Engage with clients, collaterals and their support systems on a bi-weekly to once a month basis. Assess overall care needs of clients and collaborate with support systems to identify areas for resourcing and care management
B. Document contact with each client no less than once every 30 days.
C. Monitor symptoms, current mental status, and clinical service needs for clients on case load. Coordinate information with all relevant providers.
D. Provide case management consultation to all providers of service including but not limited to family members, caseworkers with Human Services, probation and parole officers, primary care physicians, and clinical providers/practitioners.
E. Expected to demonstrate good communication and professionalism with client, co-workers and outside agency personnel in order to provide high quality services and to enhance community relationships.
F. Complete and follow through with financial and clinical information necessary to ensure reimbursement for services provided to consumers.
G. Expected to follow all ADMHN clinical policies and procedures as applicable to this position, including those related to confidentiality, release of information and record keeping.
H. Complete specified trainings within the first 6 months of hire and all annual trainings as required by supervisor.
I. Work in partnership with other team members in a multidisciplinary approach in order to efficiently meet the needs of clients on all team members’ caseloads.
J. Participate in a) ADMHN meetings and trainings as directed, b) team meetings c) individual and group supervision.
K. Perform other duties within the scope of the position and within the experience of the employee as may be requested or required.
