The Care Manager position will provide coordination of care to members.


JOB DUTIES/ACCOUNTABILITIES: The following duty statements are illustrative of the essential function of the job and is not intended to be an exhaustive list of all duties, responsibilities and/or qualifications associated with the position.

80% Coordination of Care

  1. Provide coordination of care to members via phone and face to face interactions:
  • Acts as a representative, presenting a positive image for the company
  • Communicates with diplomacy and maintains confidentiality on sensitive information at all times
  • Stay informed on state mandated policies and procedures
  • Verify resources, services are current & up-to-date
  • Problem solve and offer solutions to member concerns and access-to-care issues as they arise
  1. Provide coordination of care in the following areas:
  • Member Engagement/Collaboration
  • Provider Referrals / Termination Notification
  • Community Mental Health Center Info
  • Utilization Management Information
  • BHI Services Info & Resources and Referrals
  • Medicaid Information
  • Non-BHO Resource Information
  • Other

20% Documentation and administrative support responsibilities :  

  1. Document coordination of care activities in Electronic Health Record
  2. Complete Member Call Log
  3. Log data in appropriate formats
  4. Support MSO team with administrative tasks:
